Please bring in your own NHS card completed and signed. If you cannot find this then the surgery will provide the necessary forms.
In most cases your medical records should arrive in the surgery within 4-6 weeks.
Following feedback from you, we are pleased to announce that we now have the registration documents available for you to download in Microsoft word format directly from this site.
To do this, click below on either the green or red icon depending on your preferred choice and choose ‘save’. Once the file has downloaded you will need to print out and complete one copy for every member of your family over the age of five who wishes to register with us. The completed form should then be returned to either the Green or Red Practice at either Waterside or Blackfield Health Centres, depending on your preference of practice and location.

It is essential that we have up to date contact details for you at all times as it is possible that you will need to be contacted after the surgery has closed in the event of abnormal blood test results for example.
